how is it that people can get their head into their avvy?

You need to host the image on another site (e.g. mine is on my geocities account).
Then go into your profile and go to Forum Profile Information. All you need to do then is put in the URL for your avatar.

It seems that avatars must be a maximum of 65x65.

I know how to get an avvy. I just dunno how to get a pic of the head.

indeed, it does. Just press Print Screen (Prt Sc) on your keyboard when you have the angle you want in Toribash Head Preview. Then paste it into your favourite art package and do what you want.
