How do I make heads? is it just a simple photoshop or MSPaint operation, or is there something more to it?

Simple photoshop. In your heads folder inside the Toribash folder open one of the heads up with Photoshop (you could probably use MSpaint too).
Then after you have changed it, save it as the name you usually use ingame.
